Abstract

Pulmonary tuberculosis (TB) is a major global health problem causing significant respiratory distress, including dyspnea. Effective dyspnea management is essential to improve patient comfort and oxygenation. One potential non-pharmacological intervention is orthopneic positioning, but its implementation is not yet optimal in the clinical setting of the study site. This study aimed to determine the effect of orthopneic positioning on reducing dyspnea in pulmonary tuberculosis patients at Royal Prima Hospital, Medan. This quantitative study used a quasi-experimental one-group pretest-posttest design. The study was conducted from January to February 2025. The sampling technique used total sampling, with 37 pulmonary tuberculosis patients as respondents. The research instrument used a respiratory frequency observation sheet. Data analysis was performed using univariate and bivariate one-sample t-tests. This study found that respondents were predominantly aged 40 years (51.4%). The statistical analysis showed a significant decrease in the average dyspnea level, from a mean of 22 in the pretest to 3 in the posttest. The p-value was 0.000. It was concluded that the orthopnea position significantly reduced dyspnea in pulmonary TB patients. It is recommended that the orthopnea position intervention be implemented as a standard operating procedure in the nursing care of pulmonary TB patients with dyspnea.
