Perceived Belief Pasien Diabetes Melitus Tipe 2 terhadap Pelaksanaan Prolanis

The Chronic Disease Management Program (Prolanis) aims to help patients with Type 2 Diabetes Mellitus manage their condition sustainably at primary care facilities. However, the level of sustained patient participation in the program remains suboptimal. Perceived belief is a key factor influencing patients’ decision-making regarding sustained participation in Prolanis. This quantitative descriptive study aims to describe patients’ perceived beliefs regarding Prolanis based on the Health Belief Model (HBM) framework, as well as their association with clinical outcomes and patient participation rates. The study was conducted in January 2026 and involved 55 community health center patients recruited through accidental sampling. Data was collected using a questionnaire measuring perceived benefits, barriers, susceptibility, severity, self-efficacy, and cues to action, and then analyzed descriptively. Results showed that 50.9% of patients had positive beliefs across all domains. Most patients had positive perceptions in cues to action (80%) and self-efficacy (61.8%). Additionally, 56.4% positively perceived program benefits, and 52.7% recognized their susceptibility and disease severity. However, 52.7% of patients reported barriers, particularly regarding family mobilization support and time constraints. Although perceived belief, attendance rates, and fasting blood glucose (FBG) levels were not significantly correlated (p-value > 0.05), suboptimal patient participation remains a challenge that must be addressed. Therefore, to overcome the participation barriers experienced by patients, the implementation of family-based interventions such as Family-Based Diabetes Self-Management Education and improved service accessibility are highly recommended to build patient’s perceived belief toward the program.
