Post Dural Puncture Headache (PDPH) is a common complication after spinal anesthesia, characterized by headache resulting from cerebrospinal fluid leakage through a dural puncture. One contributing factor to PDPH is the size of the spinal needle used. This study aims to compare the prevalence of PDPH between the use of 25G and 27G cutting spinal needles in patients after spinal anesthesia at RSUD dr. Sadikin Kota Pariaman. This research employed an observational analytic method with a cross-sectional approach. The sample consisted of 59 patients, including 33 who received spinal anesthesia with a 25G needle and 26 with a 27G needle, selected using purposive sampling. Results showed that 16 out of 33 patients (48.5%) with the 25G needle experienced PDPH, while only 5 out of 26 patients (19.2%) with the 27G needle developed PDPH. Statistical analysis revealed a significant difference between the two groups (p = 0.017). The conclusion is that using a 27G spinal needle significantly reduces the incidence of PDPH. These findings are expected to serve as a reference for choosing safer and more effective anesthetic tools.
