A country’s health status can be assessed through child morbidity and mortality rates. One effective promotive and preventive effort to reduce infant morbidity and mortality is the immunization program. Infants who do not receive complete basic immunization are at high risk of contracting infectious diseases such as measles, polio, diphtheria, pertussis, and hepatitis B. This study aimed to determine the relationship between education, knowledge, and access to health services (independent variables) and maternal compliance in providing basic immunization to infants (dependent variable) at Sumaja Makmur Public Health Center in 2025.This study employed a quantitative analytic survey design with a cross-sectional approach. The research was conducted at Sumaja Makmur Public Health Center from April to August 2025. The study population consisted of all mothers with one-year-old infants, totaling 64 respondents, selected using purposive sampling. Data were collected using structured questionnaires and analyzed using correlation statistical tests with a significance level of 0.05. The results showed significant relationships between education (p-value = 0.014), knowledge (p-value = 0.026), and access to health services (p-value = 0.028) with maternal compliance in providing basic immunization. This study concludes that education, knowledge, and access to health services play important roles in improving maternal compliance with infant immunization. It is recommended that health centers enhance continuous health education programs and improve access to immunization services.
