The Maternal Mortality Rate (MMR) in Indonesia remains relatively high, and one of the contributing factors is the low compliance of pregnant women in attending Antenatal Care (ANC) visits. A minimum of four ANC visits (K4) is an important indicator for early detection of pregnancy complications and prevention of risks that may endanger both mother and fetus. This study aimed to determine the relationship between knowledge, husband’s support, and parity with pregnant women’s compliance in attending ANC visits at Kutarya Public Health Center, Ogan Komering Ilir Regency in 2025. This research used an analytic survey design with a cross-sectional approach. The population consisted of all pregnant women in trimester I–III who received antenatal care services at Kutarya Public Health Center. A total of 83 respondents were included using a total sampling technique. Data were collected using structured questionnaires and analyzed through univariate and bivariate analysis using the Chi-Square test with a significance level of 0.05. The results showed that most respondents had good knowledge (53%), received husband’s support (54.2%), and were multiparous mothers. Bivariate analysis indicated a significant relationship between knowledge and ANC visit compliance (p = 0.007; OR = 3.882), husband’s support and ANC visit compliance (p = 0.015; OR = 0.294), as well as parity and ANC visit compliance. In conclusion, knowledge, husband’s support, and parity are associated with pregnant women’s compliance in attending ANC visits. Strengthening health education for pregnant women and increasing husband involvement in antenatal care are important strategies to improve ANC compliance and support efforts to reduce maternal mortality
